A friend of mine has helped building a VN recommendation site to introduce beginners to visual novels, or to just recommend some (hopefully) new stuff to people who already like VNs as well. I tried out the recommender quiz, and it looks pretty decent! You can very easily nudge the parameters to make it recommend you things for specific genres and limit it to eg. only VNs available in English so you don't have to filter through untranslated ones.

It seems the closest this site gets is that it marks certain VNs as 18+ at the moment, but I can ask my friend if they've considered this, or if they'd consider adding content warnings.

I talked with them a bit, and they are definitely open to adding content warnings, but because it will take some time and a lot of effort to implement they'll be waiting a bit to see if the site ends up being used enough to warrant updating it :)

They got back to me and have updated the site to display content warnings if they have been added to vndb (from which the recommendation site pulls its data), it's maybe not exactly what you're looking for but it should warn for the most common things people might want to avoid.

It has an abuse plot you can opt out of, but only if you know enough about how VNs conventionally signpost route choice
